Free advance directives forms and Free Advance Directives Final Exit Network partnered with the U.S. Living Will Registry to provide members with free access to advance directives kits. Click here for state-by-state listings of living wills and durable powers of attorney for healthcare—also called proxies or surrogates.

If you have difficulty accessing these documents, write to the Network, and we will print and mail copies of the documents you need directly to your home. Free Registration Final Exit Network’s partnership with the Registry – as a Member Community Partner – ensures that your advance directives and emergency contact information are available when you need them, wherever you are. The U.S. Living Will Registry is a nationwide service that stores your final wishes electronically and makes them available 24 hours a day to healthcare providers across the country. Your advance directives—living will, healthcare proxy, or both—are made available to your family and doctors when you need them most: when you’re too sick to communicate your wishes. Registered Members receive notification by mail, along with labels to attach to your driver’s license (or photo ID) and health insurance card stating that you are registered with the U.S. Living Will Registry, and a wallet card listing your Registration number. Once registered, you are registered for life. You will have peace of mind knowing that your wishes will be available whenever and wherever they are needed. Your family will not have to make difficult decisions on your behalf, and by having your wishes available, the problems that can occur when family members disagree about treatment will be avoided. Final Exit Network is excited to offer this valuable service to our Members only at no charge as a benefit of membership. Network Members who wish to register their advance directives with the Registry should contact the Network to request the Network’s official “source code” which will give you the authority to submit your documents to the registry through the Network’s partnership with the Registry.
